Per a post from Josh Riggs on the Peach State Roads Facebook Group (closed group, so I included some relevant quotes):
SR 204 was truncated north of SR 21 (DeRenne Avenue) and SR 204 Spur, from SR 204 to Skidaway Island, was eliminated altogether.

So does 17 now turn north at Bloomingdale and cross I-95 to end at 21 Alt?

Can you post news articles or any information from GDOT?

Just have the posts on the Peach State Roads FB group to go off of, plus the photos Josh Riggs posted. I directly linked them below, which appears to work when I try them in a different browser where I am not logged into FB.

So, SR 17 will be removed from Bloomingdale Road and placed on the Jimmy DeLoach Parkway extension?

Will any changes be implemented on SR 21 Alt.?

So, where exactly does this SR 17 Conn. go?  All the way to US 80/SR 26? Or does it turn before that?  Has SR 17 been truncated currently?  What is the time table for all of these changes?
