UT 308, in Kodachrome Basin SP near Cannonville, is not in the HB. Based on a post over in AARoads, I found GMSV imagery (https://www.google.com/maps/@37.5015903,-111.9937149,3a,15.8y,51.87h,93.57t/data=!3m8!1e1!3m6!1sAF1QipPvng2cK1GHOBSPz6g0nqheD1iBx11GC8hIPfmQ!2e10!3e11!6shttps:%2F%2Flh5.googleusercontent.com%2Fp%2FAF1QipPvng2cK1GHOBSPz6g0nqheD1iBx11GC8hIPfmQ%3Dw203-h100-k-no-pi-0-ya23.24313-ro-0-fo100!7i8704!8i4352) showing that the route is signed, at least at one end. If it was left out of the HB as unsigned, now it's signed, and can go in.
I looked at the thread for usaut over in the CHM forum, no mention of UT 308 or why it wasn't included.

Also, is NickCPDX still the active Utah/Nevada maintainer? I don’t think there have been any changes to either of those states since last October.

He's not very active, though he has done some work on the state.

That sign is either left over or installed in error - UT 308 was officially decommissioned on March 21, 2003 (https://www.udot.utah.gov/main/uconowner.gf?n=200609211021191).
It should also be noted that there is no SR 308 in the online GIS map from UDOT (https://maps.udot.utah.gov/uplan_data/documents/HRO/).
Based on this I'm leaving UT 308 out of the HB.
