It appears that MA38 has been extended from MA28 to MA99.

Interesting. There's some signage southbound from MA28, though it disappears at the I-93 ramps. I found nothing northbound, including at the Sullivan Square interchange, which is poorly signed; nothing for MA99 either.
Perhaps most telling is, despite the continuing southbound signage, milepost 0.0 (https://www.google.com/maps/@42.3929769,-71.0857875,3a,26.1y,170.49h,89.78t/data=!3m9!1e1!3m7!1sbn6X7SwsgfHlQ3NamqgE0Q!2e0!7i13312!8i6656!9m2!1b1!2i41).
This reminds me of the semi-recent AARoads discussion, Extension of MA 2A? (http://www.aaroads.com/forum/index.php?topic=10141.0)
As Jim noted in that thread,
I looked into this a bit more for CHM purposes. The 2012 MassDOT Road Inventory shapefiles still show 2A ending at Commonwealth.
Similarly, 2012 MassDOT Road Inventory shapefiles still show 38 ending at 28.
I may try to download newer shapefiles and see if anything's changed, but it's low priority.

New shapefiles (http://geo-massdot.opendata.arcgis.com/datasets/road-inventory) dated 2017-07-11, and MassDOT's Road Inventory (http://gis.massdot.state.ma.us/maptemplate/roadinventory) page agree with a terminus at MA28.
I'm going to accept the consensus from AARoads, that of an unknown local agency going rogue and playing fast & loose with signage.
