Title: SC: I-85 exit number issues

95 -> needs recentered due to being shifted to a new bridge to the west of the old one. https://goo.gl/maps/EHGGzi5QLqJBxz1q8
97 -> *97 (is no more)
98 -> *98 (is no more)
104 -> needs recentered due to new overpass location
106 -> needs recentered due to new overpass location (also effects US-29)

NOTE: US-29 needs to be realigned around 106 as well, as this was a significant realignment for it, and might be worthy of a news entry.
