I couldn't find any signs outside Rhode Island, save one that's not on the mapped route in New Jersey. That also goes for the whole route down to Yorktown.

I definitely saw a sign on US 1 southbound somewhere near Dumfries, VA, in 2014. I have no idea exactly where it is or if it's still there

While there are some gaps in signage, the route has numerous postings in Virginia.  The state defined route as of 1999 can be found here - http://www.vahighways.com/route-log/VDOT%20logs/99_pg56-57.jpg.  The current NPS route is discernible when zoomed way in...there are definite discrepancies with the posted route, even with the newly installed signage.

They are replacing green signs with brown signs that are harder to read at speed.  Some green signs are just gone now and a few instances exist where a loop road is no longer connected on both ends to use the route (SR 695 at Bowling Green for example)

Looks like a LOT of routes haven't been adjusted to show concurrences correctly - in fact, it's nearly as rough as when it first appeared as an Easter egg last year, at least in NY.

That was handy, though I think my main problems were looking at Alexandria and at the Colonial Parkway and looking for the brown signs, rather than green signs with the name. And, having got to VA last, I was being rather lazy about it.

That looked followable to me other than the TOTSOs at Mary Adams (and at both ends of that its cul-de-sacs)

Perhaps, though it fits the routing given in the VA log, even though other sources route it on the Colonial Parkway.

A few notes that I picked up from when I was updating my .list file:
-The concurrency between BL 94 and the LHCT isn't noted because the intersection points (I-94BL in LHCT, HanSt in I-69BLPor and I-94BLPor) don't match; the LHCT ones are more accurate.
-The National Road in MD, as mentioned, lacks a point where MD144Bal ends (How/Bal in MD 144Bal); it could also use a point at Martin Luther King Jr Blvd as that is where I-395Was comes out.
-The Seaway Trail has numerous broken concurrences in New York, along with one that shouldn't be there with I-190 (as it stays on the ramps and never follows the I-190 mainline).
-Does the Ohio River Scenic Byway follow I-71 in Cincinnati?  The story map on ODOT's website suggests otherwise.  Might be a good idea to double-check LECT as well, though in that case it's probably a difference in vehicular and pedestrian paths.

The southern end of MI 25 is along the Lake Huron Circle Tour in Port Huron, but there is not a shared point between the two. Does that need to be fixed?

The Ohio River Scenic Byway is missing a point just north of I-70, at the junction with US 40 (and the southern junction with US 250).
This looks deliberate, since OH7 also lacks that point. Might be a 1PPI case though.

Right, since the ORSB and LCT follow OH 7, there's no point because OH 7 doesn't have a direct connection to US 40.

The same one, it has all the byways.  I noticed it off I-90 and then realized that a lot of that was on multi-use paths, so the question is how much of the vehicular route is on local roads (where they exist) and how much is on I-90.

I think the one on the ODOT site is the Lake Erie Coastal Ohio Trail, which is different from the Lake Erie Circle Tour. Here are the two signs. I don't think we're mapping the Coastal trail, and the ODOT site ignores the Circle Tour.

In Idaho, there are a couple of broken concurrencies along US 30, through Burley and east of Lava Hot Springs, with the California Trail and Oregon Trail.
