Watching this at 11:21, it seems that NY 13 now follows Port Watson to Church to Clinton, US 11/NY 41 follows Port Watson to Church to Clinton (with NY 222) before heading north, and NY 222 follows Clinton east with US 11/NY 41 to Church.
I am sure the New York peeps will clarify if I am stating this correctly.
A quick GSV look seems to clarify this.

Street view shows that it's been like that since forever, so I'm not sure why TM has it differently. The RIS Viewer and Traffic Data Viewer have a third way (up Church and Grant), but it's largely not supported by signage.
What a mess. Following signage is probably the best way here.

IIRC usany was the first state system drafted for CHM. We used Google Maps at the time, and its accuracy was not as good back then, c. 2009-2010.

AFAICT, the routes for 11/41 are signed different NB and SB.

Haha noticed that too. Gotta wonder if there's some miscommunication going on between NYSDOT and the city of Cortland.
